Log:
Make some user manual updates:

 - Integrate custom entities into mainline text; none of these should ever
get replaced, and only had one or two instantiations in any case.
 - Move lists of supported object formats, etc. to Running Yasm chapter;
update.
 - Document BITS 64.
 - Shorten some section names and create some subsections to reduce the
section name length so it will fit in the dblatex page formatting.
